ICAP welcomes all children under the age of 17 to participate in its e-Art contest. Parents/guardians should be informed of participation any submissions are assumed to be done with parental consent. The information on this form is held confidential by ICAP. Should you win; the picture of your artwork will appear on the ICAP website. Your age, town, state, country and CAP project (if applicable) will appear with your picture. In order to protect your identity, your name, will not appear with your picture.

Parent/Guardian’s permission:
By submitting this artwork, I, as the parent/guardian of the artist agree to permit him/her to send the attached artwork to ICAP for the purpose of submitting it to the CAP e-Art contest. Should this picture be selected I understand and permit the picture to be displayed on the ICAP website only. It will not be used in any other way by ICAP without my permission. I also understand that my child’s name will not appear on the website, that I will not be paid for this picture and I may not revoke my permission.
